Chronic condition management plan by telehealth
A chronic condition management plan is a plan you make with your GP to manage a long-term health condition. It sets out your goals, the care you need and any allied health referrals. Consults are by phone or video with an Australian-registered GP.

What is a chronic condition management plan
A chronic condition management plan is a written plan you make with your GP to manage a long-term health condition. It sets out your goals, the actions you'll take, the treatment you're likely to need and any allied health services you may be referred to.
On 1 July 2025, Medicare replaced the GP Management Plan (GPMP) and Team Care Arrangement (TCA) with the GP Chronic Condition Management Plan (GPCCMP). Many people still call it a chronic disease management plan. At bulkbill.doctor, plan consults are by phone or video with an Australian-registered GP.
What happens in your consult
Your GP will:
 | talk with you about your condition, history and how it affects your daily life |
 | agree on health and lifestyle goals with you |
 | set out the actions you'll take and the treatment you're likely to need |
 | discuss referrals to allied health, such as a physiotherapist, dietitian or podiatrist |
 | check your medicines and any tests that may be due |
 | agree when your plan will be reviewed |
You'll be offered a copy of your plan. Book the Chronic Condition Plan appointment type so your GP has the extra time this takes.
Medicare and your plan
Medicare eligibility rules apply. We'll confirm at booking or during your consult whether your plan can be bulk billed.
If you're eligible, Medicare can pay benefits for up to 5 individual allied health services each calendar year under a plan. Allied health providers set their own fees, so ask them about any gap.

FAQ
Is a chronic disease management plan the same as a chronic condition management plan?
Yes. Since 1 July 2025, Medicare's GP Chronic Condition Management Plan (GPCCMP) has replaced the GP Management Plan (GPMP) and Team Care Arrangement (TCA). Many people still say chronic disease management plan.
How many Medicare allied health visits can I get with a plan?
If you're eligible, Medicare can pay benefits for up to 5 individual allied health services per calendar year. The services need to be part of your plan, and you need a referral.
Who qualifies for a chronic disease management plan?
Plans are for people with a long-term condition, such as diabetes, asthma or heart disease. There's no fixed list, so your GP will advise whether a plan suits you.
Can I get a chronic condition management plan by telehealth?
Yes. bulkbill.doctor GPs prepare and review plans by phone or video telehealth, where clinically appropriate.
My GP management plan is from before July 2025. Is it still valid?
Yes, for now. Plans put in place before 1 July 2025 can be used for allied health services until 30 June 2027. After that, you'll need a chronic condition management plan.
Is a chronic condition management plan bulk billed?
Anyone can book. Medicare bulk billed if eligible. Medicare eligibility rules apply, and we'll confirm at booking or during your consult whether your plan can be bulk billed. If you're not eligible, a private fee applies.
Can I choose my own doctor?
Yes. When you book, you can choose the doctor you'd like to see.
How long is a chronic condition management plan appointment?
It's a 40-minute phone or video consult, so your GP has time to go through everything with you.
